The aim of the Council’s Transition team is to make the transition from young people services to those for adults as smooth as possible. By joining this team, you will have a positive impact on the lives of many young people across west Cheshire as they look ahead to adulthood.
The Transitions team is made up of a Children’s Transition team, Adults’ Transition team and Autism Intervention Service, all working closely together to achieve the best outcomes for young people.
The Children’s Transition team will help put plans in place for young people and look at those who will benefit from further support in the future, working alongside the Adults’ Transition team to find services that are right for each person.
The Autism Intervention Service focuses on young people with autism who don’t have a moderate or severe learning disability and are going through the transition to adulthood, addressing any gaps in the support they receive by helping with earlier interventions, life skills training, sensory adaptions and improved information, advice and guidance.

You’ll be joining a Council that had a ‘good’ rating in a recent Ofsted inspection and received a positive peer challenge from the Local Government Association (LGA).
